Um diálogo entre a psicanálise e a psiquiatria de ligação sobre a ansiedade de morte: A propósito de um caso clínico

Resumo

A ansiedade de morte tem sido um aspeto pouco abordado na teoria psicanalítica ao longo da sua história. Contudo, verificou-se recentemente um interesse renovado neste tema, que se concretiza por um movimento nascente de aproximação entre o pensamento psicanalítico e a Psiquiatria de Ligação com atuação nos Cuidados Paliativos. De forma a validar este esforço integrativo, apresenta-se o caso de uma mulher de 32 anos diagnosticada com adenocarcinoma gástrico em julho de 2019 e acompanhada por Psiquiatria de Ligação em Hospital Geral entre outubro/2019 e Novembro/2019, data da sua morte. Faz-se uma reflexão sobre a ansiedade de morte demonstrada pela doente, que, no último mês, foi acompanhada pela expressão de afetos que eram intoleráveis, causando impacto nos técnicos pelos comportamentos disruptivos associados. Numa visão psicanalítica, pretende-se demonstrar a importância desta ansiedade ser contida no seio da relação terapêutica e da necessidade de uma supervisão que atenda à contratransferência e às identificações inconscientes estabelecidas entre os técnicos e o doente. Para isso, recorre-se a alguns conceitos da obra de Wilfred Bion considerados relevantes na compreensão e gestão do caso, como função-alfa, identificação projetiva, modelo continente-conteúdo, entre outros.
